404 Page: Fix the missing 404 page

Fixed: The 404 (not found) page is not the specified 404 page

Current Permalink is no longer “Plain” (default: /?p=123).
The 404 page should work as expected.

Fixed: Display the specified message even if it’s a Server level 404

Fixed:

For any 404 cases, display the specified message.
Display it without the Header and Footer when it’s a Server level 404
