Add Faculty’s Full Name to the existing Professional (or Licenses & Degrees) line on the Online Request Form.
There are TWO Clinic Practitioners’ pages:
One for San Joe, the other, Santa Cruz
The Full Name should be added to both pages of both languages.

As per your request, I have done this task.
Note : While doing this task, I noticed that the text contains a string ‘Array’. Please refer to the screenshot below.

This generally happens when any dynamic variable which contains an array is going to be displayed as a string.
Do I need to take a look at this also?
